Abstract

One of the lifting and transport equipment frequently used in companies is the Overhead Crane. Improper use can result in a high risk of accidents. Work accidents in the operation of Overhead Cranes are a serious risk in industry, especially in the manufacturing sector. This happened to PT DHJ which experienced 34 cases of work accidents throughout 2024. This research was conducted at PT. DHJ with the aim of identifying hazards, assessing the level of risk, and designing prevention strategies using the Hazard Identification and Risk Assessment (HIRA) and Hazard and Operability Study (HAZOP) methods. Of the 8 main activities, based on the HIRA analysis, 11 potential hazards were found with details of 18% high risk, 55% medium, and 27% low. The highest risks are found in load placement activities, electrical inspections, and supervision during operations. Based on the HAZOP analysis, deviations such as overloading and brake system failure, as well as non-compliance with procedures, were found. The proposed control strategies include engineering, OHS training, implementation of SOPs, and the use of PPE based on the principle of the hierarchy of risk control. This research is expected to be a reference in improving occupational safety in the operation of heavy equipment in industrial environments.
